1. Klinikal na Datos
Ang grupo ay may 43 pasyente, 19 lalaki at 24 babae; ang youngest ay 7 taon, ang oldest ay 24 taon, average age ay 15.5 taon; ang shortest duration ay 3 buwan, longest ay 2 taon. Lahat ng pasyente ay nagpakita ng mga sumusunod: chest tightness, shortness of breath, dizziness, palpitations, fatigue, and lethargy. Ang EKG ay nagpapakita ng ST-T changes, T wave na low or inverted, ST segment depression greater than 0.05 mv, premature beats, at conduction block.
2. Paraan ng Paggamot
Gamit ang Jing Ji Tang: Gua Lou, Dan Shen, Chi Shao, Fu Ling, bawat isa ay 15g; Zhi Ke, Bo Zi Ren, Ku Shen, Tai Zi Shen, bawat isa ay 12g; Ban Xia, Tian Nan Xing, bawat isa ay 10g; Sheng Long Gu (first decoct), Sheng Mu Li (first decoct), bawat isa ay 30g. Isang dose bawat araw, hatiin sa umaga at gabi. Isang cycle ay 3 linggo.
Pagdaragdag ayon sa sintomas: Kung ang chest tightness at shortness of breath ay malala, may maraming white sticky phlegm, idagdag ang Zhi Ke 12g, Ding Li Zi 6g, at tanggalin ang Long Gu at Mu Li; kung kasama ang yang deficiency, cold limbs, chest tightness at shortness of breath, idagdag ang Zhi Fu Zi 10g, Gui Zhi 12g, at tanggalin ang Ku Shen; kung kasama ang blood deficiency, palpitations, pale complexion, idagdag ang Huang Qi 30g, Dang Gui 15g, at tanggalin ang Ban Xia; kung kasama ang yin deficiency, palpitations, restlessness, insomnia, idagdag ang Mai Dong 20g, Suan Zao Ren 15g, Wu Wei Zi 10g, at tanggalin ang Chi Shao at Tian Nan Xing.
3. Resulta ng Paggamot
Maganda (nasira ang chest tightness, shortness of breath, palpitations, dizziness, fatigue, at ang EKG ay bumalik sa normal) - 28 pasyente; Maganda (nasira ang chest tightness, dizziness, fatigue, at ang EKG ay basic na normal) - 11 pasyente; Hindi maganda (walang pagbabago sa sintomas at physical findings, ang EKG ay hindi bumalik sa normal) - 4 pasyente. Ang rate ng magandang resulta ay 65.1%.
4. Klasikong Kaso
Si Liu, lalaki, 36 taon, first visit noong Marso 7, 1995. Rekomendasyon: Chest tightness, shortness of breath, palpitations, irritability for more than 4 months, worsening in the past 2 weeks. Accompanied by fatigue, excessive white sticky phlegm, poor sleep, restless sleep, poor appetite. Check EKG: ST-T segment changes, sinus arrhythmia, frequent atrial premature beats. Diagnosis: Viral myocarditis. After antiviral, myocardial nutrition, antiarrhythmic treatments, no significant improvement, came to our department for consultation.
Upon examination, the tongue has thin white coating with thick greasy root, diagnosed as lung and heart qi stagnation, phlegm and blood stasis obstructing the channels. Treatment: Promote lung, relieve depression, activate blood, transform phlegm, open channels. Use Jing Ji Tang with additional Zhi Ke 12g, Ding Li Zi 6g. Decocted daily, taken twice a day. After 6 doses, chest tightness and shortness of breath greatly reduced, other symptoms also improved. Since the medication matched the condition, continued with modifications for another 28 doses. All symptoms including chest tightness, shortness of breath, palpitations, irritability, fatigue were resolved, patient felt energetic, appetite significantly improved. Follow-up EKG was normal. To consolidate results, continued with the same formula for another 10 doses. Follow-up until now, no recurrence.
5. Kakaibang Kaisipan
Viral myocarditis belongs to the category of "palpitation," "zhengchong," and "chest bi" in Traditional Chinese Medicine. The cause is weakened vital energy, followed by invasion of external pathogens, toxins entering the body through weakness, lodging in the heart and blood vessels. Over time, damage to the spleen leads to dampness accumulation forming phlegm, qi deficiency impairs blood circulation, resulting in qi stagnation and blood stasis. Phlegm and blood stasis interlock, obstructing the heart vessels, preventing smooth qi and blood flow, leading to palpitations and zhengchong. Treatment should focus on promoting lung, relieving depression, activating blood, removing stasis, and tonifying qi and strengthening the spleen.
The Jing Ji Tang formula includes Gua Lou, Zhi Ke, Ban Xia, and processed Tian Nan Xing to promote lung, relieve depression, open the chest, dissolve phlegm, and clear the mind; Dan Shen and Chi Shao activate blood and remove stasis, nourish blood, and move qi; Tai Zi Shen, Ku Shen, Fu Ling, and Bo Zi Ren tonify qi, strengthen the spleen, calm the spirit (Ku Shen has antiarrhythmic properties); Long Gu and Mu Li are heavy and tranquilizing, calming palpitations without trapping pathogens. The entire formula has functions of promoting upward, clearing middle, and draining downward, enabling smooth qi and blood flow, eliminating phlegm, restoring health, treating both root and branch, leading to recovery.
