Constitutional obesity refers to individuals who have been obese since childhood, with increased number and size of fat cells distributed throughout the body, also known as adipocyte-type obesity. Acquired obesity occurs in individuals aged 20–25 years onward due to overnutrition and genetic factors, primarily affecting the trunk area, where fat cells are enlarged but not increased in number, thus called simple adipocyte hypertrophy obesity.

TCM has detailed discussions on obesity. The famous Ming dynasty physician Li Dongyuan wrote in "Treatise on the Spleen and Stomach": “When the spleen and stomach are strong, one eats well and becomes fat; when both are weak, one cannot eat and becomes thin or eats little yet still gains weight; even if fat, limbs remain weak, indicating full spleen and excessive pathogenic Qi.” When both spleen and stomach are strong, excessive eating and drinking lead to accumulation of fats inside the body, stagnation of qi and fluid metabolism, internal generation of phlegm and dampness, causing illness. When both spleen and stomach are weak, gastric deficiency leads to poor appetite, while spleen deficiency results in poor transformation, insufficient production of essence from food, which turns into phlegm turbidity and accumulates under skin and membranes, forming obesity.
