When it erupts externally on the skin, rashes appear bright red with oozing fluid. In long-standing cases, repeated episodes deplete yin-blood, disrupt qi-blood harmony, generate dryness and wind, leading to malnutrition of the skin, roughness, thickening, and prolonged difficulty in healing. Overall, this condition arises from damp-heat, blood-heat, dampness obstruction, and blood dryness. Clinically, treatment should be based on patient’s constitution and symptoms, selecting formulas and herbs accordingly, avoiding rigid adherence to fixed prescriptions.
The differentiation of rash characteristics—dampness, heat, and wind—is crucial for treatment planning. The distribution, color, and nature of skin lesions serve as key diagnostic indicators. For example, rashes on extensor surfaces of limbs and back belong to yang; flexor surfaces and abdomen belong to yin; head, upper limbs, and upper trunk mostly indicate wind; lower limbs and genital areas mostly indicate dampness. Combined with changes in pulse and tongue, comprehensive analysis is required: red tongue indicates heat in blood level; yellow greasy coating suggests heat in liver and gallbladder; white greasy coating indicates dampness obstructing spleen and stomach; deep fine pulse indicates blood deficiency, etc.
